I think this song is about leaving home, in this case Chicago because FOB is from Chicago, and leaving someone at home without really getting any closure. FOB is substituting physical pain for a deeper meaning. Probably like a girl who lived close by like a neighbor and had a fall out (No pun inteded) and has left town but is still reminded of someone he left behind but feels like he isnt responsible for why they fell apart and will never apogize, but still feels hurt.
