I am not for sure what this song is about and i am a little intoxicated right now but i think it is about the lead singer being in an accident with whoever he is writing about. in this accident he paralyized the guy and he feels responsible for the whole thing. he can "see it in his eyes" that the guy knows it is his fault. he wonders that if the guy could use his paralyized hands whether he would strangle him for doing this or not? i love this song. this is just my take on it. "The dusty dirty chair" is just another reference to the paralyized friend.
