I think this whole album works as a concept album about the stages of addiction (i.e. Twelve-Step Program). This song sounds like he's pretty desperate for something like smack or coke. Something that isn't very good for him, at least.
I absolutely agree. There's 12 songs on it, its called "13th step". It's like I've got this done, now what? Although not all of the songs talk about one single step (The Noose might), I think that the entire album follows the concept of "what we used to be like, what happened, and what we are like now", as is said in the AA preamble. The album may be arranged in order according to this rather than steps 1-12, but I believe the album is definitely themed around a 12-step program.
