This is such a nice song..I think it's about him trying to tie someone he loves to him because this person is fading or about to die, which the lines about helplessness and hospitals kind of imply. The poetic lines are so sad.."I ain't a soft and saccharine wannabe Still I pray to God, this song will end happily."

Who's the Elephant Man by the way?

The Elephant Man was a deformed human in the 19th century who people treated badly because of the way he looked. Rufus mentions him in the song probably because he feels neglected and alone like the Elephant Man.
