Its about a guy that's everything to her and everyone wants him but he only wants her then leaves he because he loves to see her break but she still loves him.
Like Courtney there are a lot of messed up layers in this song. So the first one is that when Courtney was telling Kurt that she loved him she sent him a doll head in a heart shape box. perhaps a reference to that there was also a Nirvana song that referenced this "heart shaped box"
Song probably references her troubled teenager life where she was a exotic dancer in many locations including Japan. Some people said that she was a child prositute as well but not proven. When I say she or her I mean the persona of the poem...

First Stanza: References her drug use and her use of sexuality/looks to gain her drugs. Being used as a sexual object. You look at her first and you see what sexuality and when you look closer you can tell she shoots up in her arms. That she is just a sexual object. It is also a shocking image to see a doll that has veins. Dolls often do not have heroin tracks... So this is a flawed doll when you look up close far away she is a perfect doll. Perhaps someone you can just mess around with.
Second Stanza/fifth: I think that is not about Kurt although she did heavily pursue him apparently. I think it is kind of saying yeah you are sexual object they want you. But she is saying yeah I want this life. I like that I am a woman that is sexy and can get what she wants. Also it could be someone romantic in her life that actually wants her as in romantic way that says yeah they want you but I want you in a different way... kind of a way also to psyche herself up before she does these acts. That you know They really want you (not referencing Kurt at all) almost have to keep repeating to herself to believe it...Also maybe saying okay if they say "They want you" and you say "I do too." Because they do not really want her they want certain parts of her but not actually her..
Third Stanza: She is saying she wants all the drugs/sex she can handle. In a sort of bad situation. She cannot admit she wants/loves so much. She has been so used that she cannot express real feelings. Ie she has faked it so much that she has become fake. like a doll not a woman. Real love would be scary or perhaps it is not a healthy love or choice.
Fourth Stanza: Okay so it could mean that her situation of drugs/sex could be the same as someone who fears losing her man that she gets plastic surgery. Knife: also cutting up of cocaine. She does so much drugs that she cannot hide it anymore and it affects her face. Also how women change themselves to please their men or men in general.
sixth stanza: she is saying she wants it all drugs/sex. Because Cake= can mean a kilo of cocaine. She is in a abusive relationship with a person that likes to break pretty things because he can. Trades sex for drugs. Then she fakes it with him that she loves him because she gets drugs. She is dead inside and there is someone that could love her somewhere else mentioned in the other stanza but she is with drugs instead. She does drugs to take away the pain. The "some day you will ache like I ache" I do not think is to Kurt or to any man or lover of hers. I think it is that if you are druggie, a suburban housewife or any sort of woman at time you will understand my situation.
