i've never heard the song, just read the lyrics. to me, it sounds like they went thru a really bad breakup, and he's getting the chance to see her again, after a long time away from eachother. maybe he could have been obsessed w/ her...the medicine he cant keep down, or he was depressed after they broke up. i dunno, just guessing. also it could seem like she is sick. "She looks just a little bit different now.
