I think sometimes in a relationship you can end up glossing things over, so you sometimes don't even notice anything's wrong ("I don't even care / that you were so unfair / I worshipped one time at your feet / never heard what was said"). When this happens, the end can come as a total shock ("in a flash you were gone") and then afterward you can realize what was really going on ("your words hit me just last night / when I saw it with the aftersight"). Even though it hurts ("Drown in my tears, succumb to my fears"), you have to move on ("I'm not O.K., but that's O.K.").
