I like to think of it being about a guy who is depressed and in love with some girl. He hides his his true personality from everyone else, who think of him as normal like the rest of them ("in their eyes, i was alive/a fool's disguise").
He wants to feel better, but he's stopped from doing this because of his love for the girl, which isn't reciprocated, contributing to his depression ("yeah i want something new/but what am i supposed to do about you?") Things could be better for him if he didn't have to see her on a regular...
