It seems to me that the song is about a rapist (no matter physically or mentally), as evidenced by the phrase “arms untied” the phrase about a cigarette in the bed and a knife in the heart can be literal or metaphorical, who knows. love this song.

I think this is about a girl (hence Bilinda singing) making her (ex-)lover feel like shit; she loves making him (or her) feel pain.
-She likes to see him trip, fall down, and crawl around. -Arms untied could either symbolize that they are now ex-lovers (untied), or possibly untied arms from having them tied during sex. I think the former makes more sense, as her smile is scratching his eyes out. -He stares back strangely, emotionally distraught by the blade she left in his heart. -Finally, underscoring how she doesn't give a single fuck about him anymore, she slips a cigarette BUTT in his bed. I think of this as the metaphor for their relationship, which was short-lived, like a cigarette.
