I love it that he felt feminine. I think it was because he wasn't accepted by his father as a kid. I have 3 sons and it's very important to boys to be accepted into the" mans world" in order for them to feel like men themselves. Kurt was not accepted by his father. I too believe this song is a metaphor for smoking a joint. Think of the business end of a joint in relation to the lyrics. It gets all moist from spit and when its been slightly pinched into an elliptical shape by your lips ie... tickling its circumference (which is the actual lyric not "particularly enjoy" as it says above) it resembles a vaginal opening. Sucking the walls of her anus = taking a drag. Kurt said on several occasions that his favorite drug was pot.
