This is saying "no one gives a shit about me." It's saying -- i'm a dumbass but a good person and i'd help you in the hard times. But in my hard times, no one is there to help me.
A plea for his real friends in life. He's asking who is actually his friend, and who is just acting like one.
As well as the context already provided, I also got the impression that this person who is deemed as "dirty" through his over-the-top actions is actually intending to pushing the boundaries between what is deemed as "appropriate" or "normal" behaviour, so there is a double-layer of questioning here as to the way people behave within themselves, and amongst others. The dirty drowning man is the eiptomy of "nakedness" and "truth" - these pretentious bastards who claim to be his friend (when it suits them) deem him as "crude" and "immature" by their standards, but he is actually the most reliable and moral one out there.

This is a really deep song considering that when I was a teenager I thought primus was a joke band. (they are definitely not) Well, we live and learn I guess.
I love the entire Antipop album, this song especially. Les's poetic ability is totally unreal. The words are simple to understand, but the message is buried beneath the sonic onslaught that has made Primus famous. Wacky or not, Primus is one of the last unplucked gems.
