Since it is from "The Dark Saga" album, it is a part of the story of Spawn by Todd McFarlane. (Not sure if I spelled that right though.)
At this point on the album, the hero (Spawn) is seeking to reject his new role, and he finds himself with no allies. Rebuking the devil, he is now Hell's enemy, while at the same time, as a soldier of Hell, Heaven wants him destroyed too.
