I’m think I’m taking the most literal interpretation of the lyrics, here it goes: for me it talks about someone with a multiple personality disorder or maybe schizophrenia.

All the verses, plus the not included here (the dark/creepy/gutural “Why I’m covered in blood, why are we covered in blood?”) makes me believe the song is about this mentally-ill person finally losing to its evil personality and committing murder.

I think we mostly listen to the “evil” side in most of the verses, but there are some others definitely spoken by the “innocent” one, like “ With all my mental ease they dance inside my brain” or the previous one I mentioned: “Why are we covered in blood?”
