I think this song is all about the hurt a person feels, maybe during depression, loneliness or sadness.
The first verse paints a picture of a dark thought or feeling that's continously feeding on that pain, a feeling that used to be hidden deep down.
The pre-chorus is maybe a scene of a literal or figurative sunset, the sun or light disappears and everything goes dark, leaving them with only the hurt they feel and the dark thoughts that surround it.
The chorus is then the person trying to decipher what this feeling actually is. Now that they're alone with these thoughts, they can try to attempt to deal with them somehow, though not knowing how to confront it. While coming to the realisation that they do need to confront the harsh feelings in order to move on, that being alone with the thoughts is the help they need to do so.
The second verse is this person attempting to self-reflect, while trying to deal with the thoughts, all they find is old scars, and they have to confront these too, by figuratively ripping the band-aid off and facing those feelings.
The bridge sort of represents how vulnerable this person is while in this state of dealing with their feelings, they don't know what they're meant to be feeling, or doing.
The second bridge is the apex of this experience, going through the motions of confronting the feelings, healing in the process, and that they don't want these feelings because of the thoughts they create.
The outro then leaves us with the emotional stress built by facing these feelings, getting somewhat desperate to be rid of them.
