This song is about a parents reflection of his child.

I'll start with the chorus. As y'all probably know, each person in the world shares the same DNA/genes with his/her parents ("When I look at you I see me"), but that also means that your genes will appear in your grandchildrens body, and so on. We all share the same genes as our forefathers from the start of human evolution ("We will always be part of history", "Forever I will always be").

"I close my eyes and drift away And I'm wondering what you're doing now"

Well, this part means that he's wondering what his child is gonna do when he's dead.

"Mind and soul we are the same A hand on your heart, you know I am there"

Those who grew up with their parents will always remember them and always love them, that's why we all have our parents "in our heart". But this sentence could also mean that his heart is pumping his parents blood; we have the same DNA - the same blood - as our parents.
