It's pretty straight forward. He's talking with some he was clearly friends with or possibly in a relationship with at some point in the past. He remorses on the past; talking about a time they drive fast past the other person how and another time that the other person had had to much to drink or had to many drugs and Alex happily helped them. Then he uses the collective "we" which leads me to believe the person to whom Alex is speaking did something that damaged his/ her relationships with a lot of people (based of the previous lines I'd guess drug abuse). Alex then uses the last seven lines to admitt to the change and admitt how he regrets how things turned out.
