This song is basically about letting someone go who was once very close to you and dealing with the end of that relationship.

The verses talk about missing this person ("I miss your brain..."), while the chorus talks about not having any regrets about the end of the relationship, but knowing that you lost someone who meant a lot to you. ("Still I have no regrets [...] but I feel like I lost my very best friend.") During the bridge she talks about how she will always remember that person and how he will always be a part of her life, eventhough his "picture is off her wall".

For now she still has to deal with the end of the relationship ("but I'm still waiting for your call"). At some point she talks about how he wrote her poetry, while at the end she "puts away his letters" but still "saved the best ones".

She's torn between sadness and confidence. She says she doesn't want to pretend that she isn't sad, because she knows that she lost her "best frend".

I agree with MKay's statement about Best Friend-Madonna's saddest song ever about a Brit rocker named Guy Ritchie. Madonna is indeed sad in this song-she just cries and is sad for her loss.
