I'm pretty sure this song is about his children that he adopted from Ethiopia.

The main theme of the song is that Tim Lambesis, the vocalist, is amazed that besides everything they have experienced in their homeland, they're still so strong.

In the bridge he also says "At times I'm carried on the shoulders of a CHILD", clearly mentioning the word 'child'.

When he says "My source of power is seeing the weak prevail.", he means that he's able to stay strong knowing that even his children, who experienced so much suffering, can do it.

And when he says "I could not imagine a world without you, where I looked you in the eyes and left you to die.", he means that he can't imagine he would've looked those Ethiopian children in the eyes and left them there in Ethiopia, knowing they would die.
