I think this song is about a relationship, or a past relationship, someone that consistently haunts his thoughts. 'Fly (your wings are jaded)' : she flys away now as shes grown tired 'Cry (you're enervated)' : I think he is talking about the both of them lacking the energy to put into the relationship.
'Die (for what you believe in). Try (you've nearly made it). Why...' He believes in her, and he is so close to having made it work, but he can't understand why he trys so hard to get to that point especially since it all fails.

"This is the saviour and its form...' : this line is hard, it could mean a number of things, but I think he sees her as the saviour in human form.

I think this song is talking about how we are killing ourselves and all living beings by destroying the earth. I'll explain the parts that most prove my point.

This is referring to the animals. They are dehydrated and their wings are jaded because of the negative changes that are occurring to their habitats, which in the long run affects them drastically.
"Cry (you're enervated)."
Obviously, this is how the affected feel.
"You're the plague within my dreams."
This could refer to what the animals think of us.
"Soaring through an atmosphere of an adequate lack of strength."
Refers to how we are destroying our atmosphere, which is now weaker than it used to be.
"(and we're responsible) the truth is that it will end here."
Directly states the message of the song.

Here it states that we don't want to accept the fate we'll face if this world keeps getting worse.

I think this refers to us, humans. We are the saviors (or at least we could choose to be), and our form, which is ironically affecting us negatively.

We wan't to fool ourselves in thinking that things are alright while they aren't.

Here he wants to make things right; he wants us all to admit that we are messing things up and correct our own errors and eventually fix things.

The damage is already done, like it says, and at the last moment, when things are at its worst, we realize our fatal error.

Personal opinion:
Ashe O'hara is the lyricist and vocalist of this song. Ashe is known to be openly gay and support the LGBTQIAPK community. I believe he wrote the lyrics drawing from his own experience.
This song to me is about a relationship, especially about a relationship that is not accepted by the popular opinion. It is difficult in such a relationship that one person can force himself to do things he regrets or neglect who he truly is. The lyricist urges his love to embrace their love. What has been done is in the past. The two people in the relationship may have hurt each other badly or just one way. Love has the transcendental power that helps him stop denying and save themselves.
Nocturne provides a gentle but powerful reminder to wake himself/listeners up and love courageously.
