I think this one is clearly about rehabilitation.
My guess on why it is called "day four" and why this is mentioned at the end of the song: I often read that when you are trying to wean off a drug addiction or alcohol addiction, the first three days are the worst.
I think it's about aswell. He says

He says he will stay clean, and drugs might give you a good feeling for a while ("anything it gives"), but it will backfire, and add nothing to your life or even make things worse ("anything it gives it will take back").
I don't have a clue about what the rest means, I wonder what he means with day four...

Day Four. I could never make it past day four. The first couple of days seemed easy because it was still in your system, day three was always death.. and day four was when I always gave in. And the way you count the days, the months, the years, always counting.

I definitely see the recovery and rehabilitation themes, as well as some imagery on death and resurrection. Maybe the whole Day Four title comes from the Jesus being resurrected in 3 days notion?
