I think it could be about drugs. Sally could be a street name for speed, for instance... Rereading the lyrics, there's lots of talk about burning and flames, running fast but not running away, Down the street where they offer the deals, taste from the hand that feeds, feeling something grow from a whisper to a shout with a fist in the air... it all sounds like drug use. (Check out Tori Amos' song Horses about heroin - very similar in metaphor)
To me, this sounds like it is about a childhood best friend of Kimbra's, maybe that she has grown apart from over the years. Now she's caught up in the fame of her success, had opportunities to see lots of the "real world" now, but is still the same person at heart.
If you watch this video of Kimbra back when she used her full name and wasnt so popular http://www.youtube.com/watch?v=GONPmnfeP84 it makes me think maybe this song is just about her transformation into who she is now, as an artist, and the emotions that go along with changing your image to be successful or fit a certain mold. The song Posse deals with some of the same ideas as well.
