I think the song is about someone who's been through a lot of stuff to deal with (like abuse). The part about being a guardian could mean that someone cares enough to watch over and help. I think the watchwoman part is about learning how to take care of yourself and look after yourself.

Everyone needs a guardian!
For me this song reminds me of a late friend of mine. It makes me feel as if SHE'S talking to me when I listen to this song, as if she's saying she'll be my guardian and my keeper. I guess it works both ways as well.
