It's about love and separation. He is bound to this person, though the lines of communication are blocked at times. Even when they're apart, they both feel the love and connection. Maybe they're splitting up. Maybe they're fighting. Maybe some external factor is keeping them apart. Either way they're connected on a higher level that transcends physical presence.
This is a beautiful song that I never gave full thought to until today.
