I think he's singing about a woman who is ill and knows she will die soon, the lines "You said you were quitting after your next pack" indicate she probably has cancer (also not the first time Middle East has sung about cancer) ..the relationship seems quite pressed i think, he drinks a lot and could never really tell her how he felt whereas she seems to be able to tell him everything "And I'd give to you in death what I could not in life " Last verse is what gets me every time, this is a very powerful song... Middle east writes such sad but brilliant music :')

@SukkaFish92 such a beautiful song. I think he loved her and couldn't tell/help her while she was alive. Him standing at the back with a rose in his hand is expressing what he couldn't say when she was alive.
