It sounds like a perverse and seductive addiction. Something the subject knows is destructive, and yet that is what fuels his lust and desire. There is a complete lack of love and connection, like the lyrics say, it is a possessive relationship, and she doesn't much want or even like it.
I'd say it's a song having a lot to do with rape, or a non-consensual sexual moment. Spoken in the words of a predator.
I think that "she" is his soul and the song is about ... some sort of a drug or love eventually. Here is what I see in the lyrics as meaning (maybe):

"I need to watch her abused till I'm satisfied" - obviously he is trying to kill himself or destruct his soul through drug abuse. And the reason there could be a love that left him, which is why he is also sad and angry. "You mean nothing, nothing. No you mean nothing to me." - drugs mean nothing to him, its just the reason why he does it. "Makes me feel sick to know she's had enough." - he doesn't want to satisfy his drug addiction, but moreover finish himself off with drugs. Very sad, isn't it? "I hope you know, I live tonight through methadone highs." - I think that's where he reveals what's he talking about.
But the song is definitely not about drugs, but the reason why we use them - to ease some sort of pain or weakness we cannot bear. Well, It's just a point of view, maybe the artist didn't mean nothing special after all ...
