So perhaps it isn't necessarily about him and Meredith, as we already know Annie is his mother in law...either way, this song is beautiful, and terribly sad, but that's what makes it a great song. It's one of my favorites on this album.

this song is beautiful, i think most of this album is about a relationship since it feels its a reoccurring theme in the songs.

After reading the other comments, it wouldn't be surprising if this song was about the said miscarriage of anthony's wife. although it also seems perhaps an abortion? This song opens up such a wide window of other possible meanings if we don't put it with Anthony's life. Leaving someone in a relationship because you both know it's time that it ends.
There is a term in french, "l'esprit d'escalier", which translates roughly to spirit of the stairwell. The saying is used to discribe thinking of something you should have said, but were to into the moment to think of during an argument. Could this song be based around this term? Perhaps the things someone should have said during tension over losing a baby? Whatever the song is about it is expertly written and backed up by emotionally moving music. Cheers!
in altpress a few months ago, there was a tiny paragraph talking about circa survive in the studio, and i remember reading that anthony said he spent time in like a mental hospital (i believe) after his wife had a miscarriage. today they announced they'll be on the june cover of ap so i'm sure that feature will go more into it.
i relate to this song a lot because my mom had a miscarriage, i think i was like 11 or 12 when it happened, but it's still one of the worst things that could ever happen to a person. i know anthony especially loves kid and i'm sure he wanted to start a family. :/ hopefully he and meredith will try again.
BlueSkyWind has a really good point, I can easily see this being about things said in the heat of a painful moment. I can also see it being a conciliation, after the argument, a way for him to say whatever happens he has no regrets, no hard feelings. Either way it's a beautiful song.
Annie is Anthonys mother-in-law. He said that on their live stream they did on 4/20. I think they didn't want him to say that for whatever reason. lol.
