When this song talks about the "heart" it's talking about the sinful desires of the flesh. Our fleshly desires are always deceiving us, trying to convince that we can quench the longing in our hearts with the things of this world ("Chasing love, where it can't be found"). It also says that the heart is "looking out for yourself, no matter what I say" this means that the flesh seeks to pleasure itself through it's sinful desires (read the book of Romans for more info on that). In the end it paints a brilliant picture that Jesus is the only One who can save us from our own hearts, and the only One who can fill their longing. Great Song.
