I think it's about going inside yourself and meeting something dark inside... and having it follow you out.
I didn't realize this at first but it's actually based on the horror manga, The Enigma of Amigara Fault by Junji Ito. I'd actually read it before, but it wasn't until TVBS wrote it on their facebook fanpage when I actually saw how obvious the connection was.
(Course, death to the author is always valid; I still have my own standing interpretation.)
