I agree with the 27 Club reference and that its, at the very least, partially about pete wentz, but reading about the album Pete Wentz talked about the album being less auto-biographical so i think the song is a story of a successful star which mirrors Wentz's own problems but may also be true for many stars.
Also the 27 club theme plays at 2 points in the song, "Just to feel like you" and "Chasing the direction you went"--some of the 27 club members were heavy drug users (Cobain, Hendrix...even the recently deceased Winehouse, though this song was written way before that) "your're a bottled star" could be interpreted that the stars of the 27 club are in coffins or "bottled" and "you shine in sky" theyre no longer alive so they live on in the afterlife.
My favorite song on the album by far its sung very emotionally by Patrick Stump which make the lyrics sound unbelievably great..
is it just me, or does this song remind you of Ryan Ross? (Ex-Panic at the disco lyrisist, singer, guitarist, etc) But really, this song seems to be about him, (and their friendship) For example, in the first few lines,

it seems to refer to his childhood, where he had a drunken father.
in all these lines, they refer to Ryan. Even in some, (now this may be a long shot) that may seem to be about drugs..maybe they're metaphors.

i think he's talking about how he doesn't want to do this anymore, it doesn't feel like it did before(I can't remember, the good old days) and how he doesn't get the thrill at concerts like everyone else (doing lines of dust and sweat off last night's stage just to feel like you)
I think the song is titled '27' because that's how old Pete was when he wrote it. That obviously wasn't the best year for him - "i was pretty sure id die at age 27 (glad i didnt)".

I can't believe it took me this long to realize, but 27 is a reference to the 27 Club. Many famous musicians and actors died (often of drug overdoses) at age 27. This song's about Pete's problems with prescription drugs, I guess. "My body is an orphanage, we take everyone in..." all the drugs...

I think this song is about how hes tired of being famous and playing all these shows..i think the title 27 could be referring to kurt cobain because he died at age 27 and was going through the same exact thing.
I reckon the song's about the descent into drug addiction. The line "I'd shoot the sunshine into my veins", is a reference to the way heroin feels when it's injected. The lines "And it's kind of funny The way we're wearing anchors on our shirts When being anchored aboard just feels like a curse" are referring to the way that addiction is like being anchored, because you can't move anywhere in the world (get a job easily etc) with an addiction. However, many addicts are aware of their problem, thus creating the line "just feels like a curse".
The part about the "milligrams in my head" is obviously about drugs, legal or otherwise.
"Doing lines of dust and sweat Off last night's stage" These lines are talking about snorting something, perhaps cocaine (due to it being a popular party drug and the fact that a slang word for it is "dust").
To sum up, I think it's about the fact that a LOT of famous music stars are "shackled" by their drug addiction and that they're surrounded by it all the time, therefore it's virtually pointless to give up,
for some reason i think he tried to write this from joe's perspective. this seems like how he feels.. but idk. after ioh joe especially had some hard times with the band (apparantly, from blogs and interviews) 'milligrams in my head burning tobacco in my wind', but maybe that doesnt mean anything.. anyone could relate though. thats what its all about..
