All elements of death coming around. You have the black bird tapping on the window, a reference on lore to someone going to die. You have mention of the midnight train, yet another reference to coming death. Last but not least, mad shriekin' woman (banshee) also a well know lady like figure, with her scream comes death.

The line that is left to question is the one that starts off each verse. Is this demon a feeling he has suppressed within himself that has drove him to insanity? "Sick man smiling at a stray dog in the rain" leads me to believe, that this is meant to come from the mind of a mad man, who is waiting on the coming death, and looking at all the different fabled signs that could point to it.
