I think about liking someone who doesn't return the favor. cigarettes, they only hurt you while they may give you pleasure-- in the long one every cigarette is a waste ; maybe she relates every moment she spends with this someone to a cigarette
I don't see this as a song about a relationship. To me it sounds more like people who might be, or have, contemplated suicide:

it might be about a person who is inlove and not feeling corresponded, she feels like she's wasting her time because she hasn't seen any improvement.
The first verse i think anyway when she says " what it is, it isn't fair cause Im wasting time, but it isn't my heart it isn't my fault. " She was in a relationship with someone, then she or they broke up. Then this new guy comes in, so she was getting over her 1st, but she still kinda loved the 1st guy but she didn't show it(for whatever reason). And I think 1st kept asking her back, but she already was commited to the new guy. I think its not the a favor she referring to the cigarettes, I think the cigarrettes reps the chances she didn't take, and the time she has wasted (the money she has spent). And the letters are just the fact that she never told the guy that she loved him.

and the secound around to the course, is about how she wasted the time with the other guy when she was supposed to be with the 1st guy. And maybe she did end up finding the original guy, but he found someone else, and she just wasted her chances, and and missed the chance of being with him loving hime, and the chances of telling him how she loved him...
