it seems to me there is actually a physical distance being referenced in the song but that there is an "echo" of that distance even when they are together.... she makes reference to him not really seeing her...looking through her, and her pretending to be someone else...then indicates that it is easier for her lover to say he loves her and is more generous towards her when they are apart...but when they are actually together they sit in awkward silence....
sounds a little like they never had a strong foundation for a relationship in the first place...that he doesn't really know, love or appreciate her for who she really is....when he's far away he can pretend she is a different person, and treats her more warmly...but when they are together and there is no way to pretend she is a different person - is forced to face who she really is...he cannot be warm or affectionate towards her...because he doesn't love the "real" her..he loves the one that lives in his mind...and that he carries with him when they are apart...
and of course its really clear that this causes her tremendous pain...she really want him to love who she really is...but the longer this goes on the more clear it is to her than he cannot love her...so she is lonely and sad...both when he is physically far away, and when he is physica lly present...but emotionally still far away....

I don't know what I get from this song, to me it could be so many things. Like what venushalley said, a long distance relationship; like whenever they see each other, it's weird, but when they're miles away it's easier for him to act like he still loves her, and that's the best they're gonna get.
But idk, i feel it could be metaphorical. Like she feels like she's miles away in the sense that they don't connect like they used to, and it's like they're so far away.

I been in a long distance relationship for 10+ years - hes always working miles away somewhere, and it feels like we can talk more and he can communicate better when we are apart, in fact he spends more time talking to me about things that are important when he is away, when hes home sometimes it feels like we are strangers. whats the saying 'absence makes the heart grow fonder' then again - each song always means somthing different to each one of us, its your interpretation and how it makes you feel that counts more than the artists intention.

I think its more about taking someone for granted. Realizing how much you love them when youre away from them. "You always love me more miles away, I guess were at our best miles away" conveys that meaning to me.... However "I looked in the mirror and I saw your face you looked right through me, you were miles away" and "I hear it in your voice, we're miles away" sounds like fading love, like the feelings arent there anymore...
Hey "youaintright20 "....I think your right too.. as are the first few why not. -It can be a long distance relationship, or when you dont see them as much, does it really take an affect on the people --Or also, that we take granted(which I was always-like many- in this way) and that with time or distance, thing can fade or wither away. well...we all can somehow sometimes relate to this song. Good song offcourse...thankyou Madonna...meaningful songs are where its at unless their just fun to listen to as well ;)
