I think this song is about being afraid to tell someone something. It ties in with the song "love me like you'll never see me again" but kind've contradicts that because this person can't tell this person something and they want to but don't know how. when things are left unsaid, they fade away and there are always the questions of "what if." Regret is the last thing anyone wants to experience and she doesnt wanna regret not saying something before its too late.

I think it's about how hard it's to say to someone that you love him. But she wants to say because she's afraid that he/she dies. So I dont think it's for a lover... Maybe it's for a grandmother or something...

This is probably one of the most beautiful songs Alicia Keys has ever written and recorded. I loved it. The track listing for the album lists this song as "Tell You Something (Nana's Reprise)", 'Nana' could most definitely be a grandmother, she most likely wrote this song to her grandmother.
The overall song is about spending time with and letting a certain special someone know how you feel about them (it doesn't have to be a romance it could be a family member or a friend).
