At first it does sound like its referencing fellatio, but I think it's actually about cleansing yourself emotionally.

of course they could be metaphorical, but they could also be literal meanings. i do definitely think the song is about abuse though.

this is the best song I like off his solo it's good that he is doing his own thing this song I think means maybe a bad past or how his life feels like now who knows but it's a very pretty song
