I think this song is about a person reminiscing about a past relationship..."I am wishing you well and wondering how you are". Perhaps there was some unresolved bitterness in the relationship..."And time heals but these scars keep on tearing us apart"...even though time has passed, the bad memories or "scars" that were left behind are there to remind them of it and they find it harder to forget about the pain.
