This is probably wrong but no-one else has bothered to comment...
I think it's about a disaster (a flood - maybe Katrina...) And the singer can hear people crying out from the other side of the street (it's so flooded it seems like a river now) and no-one can sleep because of what they've seen/heard. Someone is telling them to "Prepare for the flood" and even though it's obviously too late for that they're gonna try and help people.
Anyway it's an awesome song. 'Scremo boy' if you love screamo as the name implies, how come you can't spell it?
