This song is about the times when abortion was outlawed, and a woman going through medical and emotional hell because of lack of safe access.

I think the part about "there's a priest on your shoulder, don't bother with him" refers to the idea that there's a devil and an angel on both of your shoulders, like in a lot of old cartoons.
"This skin belongs to you so use it as you choose" is the devil whispering that it's okay to want these "real naked girls" that the main person has in front of him. I think the devil is who the main person is "talking to" in this song. "Dark crimson blood, it had covered the carpets, the screams were distorted" sounds like he may have raped the naked girls and then killed them, which then leads to the "old Christian judge" who "gave out fifty to life like he was handing out chocolate" because he knew for a fact that the main person had done it.
Obviously it would be "too late to flee" because he's going to jail. And he's referring to the devil being able to help him breathe through the gruesome scene because the devil had gotten him there in the first place.
