going along the lines of what maddyunderstood said about the title misleading us while the lyrics are actually incredibly literal, i would have to agree with cigarette-- this song is about his penis...jerking off.
"none of them could get me hot like this"- none of the girls he's had sex with can satisfy him as much as he can. "you're safe inside my fist"- literally in his fist. "scars on my wrist"- either actual marks on his wrist from masturbating...or he's referring to some girl who hurt him that he doesn't need because he can please himself. "all you are to me is dead skin flaking off onto the pavement."- dead skin is falling off because of the constant friction and he's saying that he would treat the girl like nothing more than useless remains of a night of pleasure.

I believe its a mix. First, it is literally talking about jerkin it (to the point where skin rubs off). He just says how much he loves jerkin it rather than having to actually get with a chick. Then he compares the girl that he banged & forgot about to dead skin, just picking her off his life and droppin her. -then it sounds like she tried to use sex to break up his band, which didn't work.

i love max. & um so it is a metaphor, fool. im lame, but hes talking about the only person off his list he wanted was the one that hurt him, & now hes over it, & showing the person they gained nothing from trying to destroy him, & theyre a thing of the past, only dead skin to max, something you dont even notice coming off of you & falling to the floor.

I think this song is a metaphor (ha) referring to his mental illness as something that was destroying him and the band but now he's over it (all you are to me is dead skin) and he's got control of it (you're safe inside my fist) but i don't get the 16 thing....
He is bipolar that is his "mental illness" but he went crazy and lost his mind and ran around new york city talking to strangers that he thought he knew.

"love like no other" refers to addiction, "got your scars on my wrist" refers to how he was killing himself with drugs. the chorus is saying that hes done with drugs and alcohol and that theyre just "dead skin." i feel like that middle part from "so heres the plan...our own apartment" means that max somehow...
