Sad how no one has posted a single thing about this band nor the album lyrics. This song is indeed "the title track" and the single but by no means is it the best song on the record, in my opinion. It does however, kick ass. The entire CD does, in fact.
This song is about a relationship with a girl! Yes. ...I think.
Since I'm the one who posted the lyrics, let me point out that the lyrics might not be "It's a call to hope and pain!" but might be "It's a call to all the pain!" Wasn't quite sure... But with the song meaning, the first seems more correct... What I do know is that the lead singer for the band described this song as a reminder that, no matter what the circumstance, whether good or bad, it will pass.
I've always thought it was "It's a call to all in pain". It seems to fit with the rest of the song more. This is the only song I've heard by the Fold but I hope to hear more soon.
Oooh–I'd have to agree with eltroyo11: After listening to it a few more times, it definitely sounds like "It's a call to all in pain," not to mention the fact that it really does jive better with what the song is talking about.

Great work by a unique-sounding band, almost on-par vocally with The Foo Fighters, from what I hear in this song, but maybe that's just me.
I don't think it's really about a relationship with a girl. Dan writes most of the songs, and I can't think of any girl that this would inspire him in a relationship type thing.
I think it's mostly about sometimes life can just kinda suck, and you can try as much as you want to fix everything, but you need to just take a minute, breathe, and try again. "Take your time, Figure it out"
