in the beginning it seems as if Nate isn't in control of himself, and that he feels he is being dictated by someone else who has a powerful hold on him. Eventually, he realises that this person isn't how he had pictured them to be, in saying that the demon lived in them. He realises that he wants to free himself, and he tries to stand up for himself but in trying to do so, the person trying to control him refuses to listen to what he has to say and ends up deserting him before they can even find out how he feels, and in the end he ends up feeling bad, wondering where the person went and why they don't want to listen to what he has to say.

Nate explains, “A lot of the songs on this CD are more personal to me than the last record was — I wrote things that no one else but me could even tell are personal. I touched on things I never explored before.” For example, “’Hopeless Host’ is about my body not being a very good host for health. ‘Piece of Mind’ is about a guy getting a lobotomy, because society is going downhill and he can’t take anymore.”

hope that helps. a lobotomy definitely explains the title. if you don't know what a lobotomy is, it's a special type of brain surgery where you basically hammer ice picks through the eye sockets into the brain. it was supposed to help mental illnesses and "moodiness." it has since been regarded as one of the biggest medical mistakes in history.
