I believe this song is about a woman who broke up with a man and left a few years ago and went on to become famous and or successful somewhere, but was never happy. She pretends everything is ok, but he knows her well enough to know that something is wrong. He tells her not to "hide behind a smile". He asks her to believe in him and let out her frustrations.
