It always worries me to think that maybe I'm not something of this world, that maybe I don't belong here, with all of these people who are so different than me. Imagine if they all lived on some planet that they thought was Earth, and I was the only one who was in the right place, and I guess I'd just send them letters from Earth.
This is from the viewpoint of someone who dosent belong on earth and sending madness and pain is like sending feeling from earth, there may some military references.

I'm not into religion or believing in god but i think the song is about a person who sees all the pain and death, war and sickness and prays to god. He doesn't belong here, because he's not a warmonger or criminal, he's a good person with faith, so he asks what would God do if he gets some Letters from Earth (showing life on Earth and what's going on)

Letters from Earth almost sounds like the apostle Paul writing his letters, except Dio is putting a sarcastic spin on it. I don't know this for sure, of course. But Dio was well-known for writing sarcastic lyrics regarding religion, especially Christianity. All of Paul's writings in the Bible were in the form of letters that he sent to various churches that he kept tabs on. People attribute divine meaning to his writings, but Dio is saying they are just Earthly writings like anything else.
Again, just my thoughts about the song. Dio is probably the greatest lyricist of our time in the Metal genre, along with being one of the truly recognizable voices. He is greatly missed.
