From the slowish start and intro riffs to the second (obvious) change to a more nervous tune. The tune slowly grows more comfortable with time as the guitar begins to enter its first solo, and then the tune speeds up to a steadily slowing pace, starting a passionate moment of arpeggio heaven. The tune suddenly erupts into a groove and loses all forms of nervousness, just letting things go. A few heavy notes and high chords later and we get into a sudden solo that can't escape your mind. Building up to what you know is coming, a climatic solo that only ends to make way for the beginning riff to start again with more of a powerful feel. Eventually the song ends into a blissful, radiant tingling as its over :)

2. Psychiatry. A delusional, romantic preoccupation with a stranger, often a public figure.
I would guess that the second definition is what is in mind here, given the themes of "Voices", the next part of "A Mind Beside Itself" trilogy, and that Dream Theater has explored themes having to do with psychiatry on other albums, most notably SDOIT.
Erotomania- believing that someone or everyone is in love with you.
Voices- this song is about schizophrenia.
The Silent Man- says that if we talk with others we can avoid these types of problems.
As for the song, it took me some time to get into it, but I love it now. The song even uses some of the other melodies that occur later on in the suite.
