This song has beautiful lyrics that are so self-explaintory. Basically, I'm assuming he followed a girl to San Francisco -- someone who ran away from him and moved on with her life.

He has one more thing to say before he can be at peace with himself, so he travels and visits her. He'll try to win her over, and even though he might have told her something wrong to drive her away -- he realizes that she simply made him better and it was such a mistake.

But he knows she's broken, and he knows he is too -- and even though this all happened, he still wants her to remember him as someone she wanted to stay.

This song kind of makes me think of suicide like lines such as, "I won't need that medicine to concentrate again" "it isn't fair to expect you to care" " this the final night to get this goodbye right" "I hope that when I leave you will still think of me" I'm not sure if thats what the song is about tho.
