I can't tell if the orgasm sounds are like... super-orgasm... or a rape orgasm... or... she's been impaled and slowly dying...
I want to say it is about rape. I mean, the woman at the end of the song is sort of a dead giveaway because I don't think that's what a very pleasurable experience would sound like. Plus there are line such as "As you sweat and you masturbate" and "your terror rises" (probably implying that she knows it's coming, but can't stop it)
This song is about a woman having a prolonged near death experience. The lyrics are sung from the perspective of Death as an entity. For the majority of the song, Death is anticipating voyaging her to the "other side", feeling the moment of her death drawing near. She, however, ignorantly goes on with her masturbation, until (presumably) the flames of her apartment fire become impossible to miss any longer, and she has a very unpleasant return to reality as she narrowly escapes.
There's a deeper meaning, a message if you will, to this song, but it's lost on me. It's definitely not meant to be taken as literally as I'm making it. Somehow her masturbation is being labeled as pious, and in fact, her sexuality seems to be what spared her the burning fate.
