I think this song is about a guy who is keeping a secrete from someone, and the lyrics are all about the battle you go through when you have a secrete and you dont want any one to know about it.
It could be about a person who is fighting the urge to do drugs because his friend does, and he promises not to tell anyone about it:

I think it might actually be paramenia, which is a disorder or irregularity of menstration.
A lot of foo fans believe this song was written about Pat Smear and the tension between the two when making this album. It kind of makes sense. Would anyone like to elaborate on this if you think it could be? I like to hear people's opinions.
I always interpreted this song as being about Courtney Love.
