The way I always looked at this one is he is beginning to question X, but he still blindly wants to believe what hes doing will "Save this world." Hes convinced hes doing whats right and will be regarded as a hero when its all done. It does show he fells remorse for the people hes killed.
And while he does love Mary, it may be what Mary does for him he loves more. "She brings the only friend that helps me find my way" and "She always brings me what I need Without I beg and sweat and bleed When we're alone at night Waiting for the call She feeds my skin" from Spreading The Disease, shows her main job is to deliver his heroin from X while he hides from the cops.
And since no one has mentioned it, the third verse is one of my favorites on this album. The lines, "I look around my room is filled with candles Each one a story but they end the same I'll hide away in here the law will never find me The walls will tell the story of my pain" He lights a candle for everyone he kills. Each story is different, but all end in death. He also writes his confessions on the walls and in My Empty Room he talks about drawing Mary's chalk outline on the wall as well.
Nicky's starting to feel a bit skeptical about his role in the revolution, though he tries to convince himself otherwise. The beginning of the climax of the album, which peaks with "Suite Sister Mary." This song is always the first I think of when I remember this disc.
I love everything abut htis song, the best one on the album in my opinion.
And I agree, this is kind of the turning point for Nicki when he realizes hes in love with Mary, and he becomes more devoted to her than Dr. X.
