I think it's about two women who were friends early on in life, and when they were teens or something around that area they experimented with drugs together. But now that they're older they're kind of disconnected. One is a housewife living a meager existence, maybe in a trailer park, with a domineering husband; and the other is now reflecting on the past and dreaming of what was and could have been.
Tori has said, "Pretty Good Year and Space Dog kind of kiss each other... Concept being, somebody that had all of these beliefs, and then just numbed themselves."(on the fansite, hereinmyhead).
I personally see it reflecting the twisted path of any situation, especially love, where we don't know how things will work or how they will end or where to go next. Its about our own lack of insight. We're racing turtles, a notoriously slow animal and an inanimate piece of fruit is winning. Thus no one is going anywhere. That's why we need the grand storyteller, Neil, to tell us what will happen next.

It's a "mushroom trip" as Tori described, but I think it is, alongside with the random thought flows, I think it is about God and the personal battle between a person and their brain.
I'd agree with the drug trip idea, because the one obvious line in this song is "so sure we were on something". But, like any good Tori song, many interpretations work.

Ok, its a little obvious, but "seems i keep getting this story twisted so where's Neil when you need him" is a refference to Neil Gaiman, and "Where's Neil When You Need Him" is the name of the new Neil Gaiman Tribute Album coming in june (?) and with Sister Named Desire By tori on it.
Yeah, this song makes absolutely no sense to me, though it's probably my favorite on the whole album. I love all of its moods. Also -- couldn't Neil be Neil Young? Weren't they friends?
"Where's Neil When You Need Him" is a reference to Neil Gaiman. He and Tori are very close friends, and she references him in many of her songs.
For some reason just today I thought about the backing vocal part (also have been used as intro to this song), "Don't hear the dogs barking" more specifically. I wanna connect it to the actions of UN soldiers in shooting at the stray dogs that scavenged the bodies of dead in the Rwandan Genocide, but this happened a lot after Tori wrote this song. This song is magical.
