Actually, the song is a play on Michelangelo's painting on the cieling of the Sistine Chapel (or Capella Sistina in Italian). In a famous part of the painting, two lovers are reaching out to each other, their index fingers almost touching. The song is about a love that could never come into fruition, recalling the hands of Capella Sistina that could never quite reach each other, leaving the lovers frozen in longing. So yeah, unrequited love, basically.

interesting idea jackalopehellrider! However the Sistine Chapel Ceiling does not have two "lovers" almost touching fingers, it is the hand of God giving Adam life. I believe the word sistine is suppose to take on a meaning such as pristine in the song. I agree with everything you said regaurding unrequited love being the focus of the song.

looking up the word sistine just revealed that it has to do with pope Sixtus, which isnt really helpful in this context
i think its about a lonley guy(Yea, without your love I'm lonely deep inside). he found a girl and is telling her that he is nothing without her and how she moves him (Sistine Smile) like something from god. i dont think the drug thing has any weight cause danzig was never about drugs or other petty things. i really like danzig and this song is one of the best.
