My interpretation of this song is that there are many people in this world who are very....fortunate, who assume that people who are less fortunate deserve to be less fortunate, (Skanky man I don't do dope I´m already down at the end of my rope), therefor, do not help, or care for these people.
In other words, they're greedy people who have these perfect lives and take them for-granted, despite the fact that there are needy people in the world who would appreciate a little help that no "Skanky Man" would ever care about.
